DESCRIPTION
Advance care planning is an important but often underutilized tool for preparing patients and families for future healthcare decisions. Yet even when plans are in place, clinical circumstances and patient preferences may evolve in ways that were not anticipated. This webinar will examine how clinicians can guide, rather than predict, by using effective serious illness communication strategies.

LEARNER OUTCOMES
Upon completion of this webinar, participants will be able to:

  1. Describe the role of advance care planning in preparing patients and families for future healthcare decisions.
  2. Apply evidence-based serious illness communication strategies when discussing prognosis, uncertainty and treatment options with patients and families.
  3. Describe how to approach common challenges in serious illness communication.

SUCCESSFUL COMPLETION
Participants will be present during the live webinar in its entirety and complete an attendance verification and feedback survey in the Academy Learning Portal within one month of the live air date.
