Health Insurance Benefits
AbilityLab is committed to offering a variety of medical benefits that provide generous coverage for both employees and their covered dependents. The plans are designed to allow employees to tailor their coverage levels to both individual and family needs, plan structure preference and monthly financial planning. AbilityLab offers medical plan choices for our employees from one of the largest and most trusted insurance providers in the country – Blue Cross Blue Shield. Employees can choose from a PPO medical plan, HMOI, and a High Deductible Health Plan with Health Savings Account. We strive to offer flexible and competitive benefits in the local marketplace at an affordable price, and realize the importance of having peace of mind that your coverage is accessible when you need it.
As part of the comprehensive health insurance coverage offered to employees and their families, AbilityLab offers dental benefits through either BlueCare PPO or Guardian/First Commonwealth DHMO. Similar to the medical plan options, the two dental plans provide employees the opportunity to select between PPO and HMO plan structures. Employees can enroll in either dental plan, without respect to their choice of medical plans, thus allowing the greatest amount of flexibility for employees to tailor their coverage levels to both individual and family needs, plan structure preference, and monthly financial planning.
We offer employees the opportunity to enroll in dedicated vision insurance coverage through EyeMed. This insurance provides members optometry coverage and discounts for vision services including eye exams, frames and eye glass lenses and contacts at both national chains and local providers.
Income Protection and Life Insurance
AbilityLab realizes the importance of providing employees a perpetual level of income protection in the event they suffer an unforeseen sickness or injury. As such, AbilityLab automatically enrolls all benefits eligible employees in Short-Term Disability (STD) and Long-Term Disability (LTD) coverage, with the premiums paid courtesy of AbilityLab. STD and LTD provides employees income replacement in the event that they become sick or injured and are unable to work.
We realizes the importance of having peace of mind that your family is protected in the event of an unforeseen event or loss. As such, AbilityLab provides all benefits eligible employees group life insurance coverage starting on the first of the month, following 30 days of employment. This group term life and AD&D insurance is administered by Reliance Standard, and provides a policy value equal to 1.5 times your total annual compensation, to a maximum of $200,000.
Upon hire, employees must initially designate primary and contingent beneficiaries for this coverage, and you may change your beneficiaries by visiting the Human Resources Department. AbilityLab employees are automatically enrolled in this coverage. There are no payroll deductions and you’ll never see a premium invoice. However, the IRS mandates that all group term life insurance policies above $50,000 be added as imputed income on annual W2 tax documentation. AbilityLab tracks the additional amounts on a monthly basis and adjusts the year-end W2.
We offer a 401(k) plan that features both an AbilityLab contribution and an employer match on your retirement savings. AbilityLab intends to make contributions to the Plan in an amount equal to 3% of your eligible earnings—regardless of whether you save in the 401(k) Plan on your own. If you decide to make your own contributions in addition to the 3% described above, we intend to contribute a match of $0.50 for every $1 you save in the 401(k) Plan, up to the first 6% of your eligible earnings. When you contribute to the 401(k) Plan on your own, you can do so through pre-tax or post-tax contributions.
When You are Eligible
You may enroll and begin salary deferrals on the 1st of the month following 90 days of employment. You must have at least one year of employment and 1,000 hours of service during the calendar year to be eligible for the Employer Match and Employer Company Contribution. Once eligible, you’ll receive the 3% employer contribution AbilityLab intends to make for every calendar year during which you work at least 1,000 hours.
