Join Our Clinical Research Registry

If you think you might be interested in participating in a future research study at Shirley Ryan AbilityLab, we invite you to become a member of our Clinical Research Registry (CRR). It’s easy! Just answer some basic questions below about yourself and your health history then your information will be forwarded to the coordinator of our research registry. You will receive a follow-up call from one of our CRR staff who can answer any questions you might have regarding the registry. If you are interested in the CRR it only takes about 10 minutes, over the phone, to consent and complete the enrollment questionnaire. Although it is our hope that enrolling in the CRR will provide opportunities to connect you with research studies, it does not guarantee that you will be matched with research participation opportunities. Consenting to join the CRR also does not obligate you to agree to participate in any future research.

All the information you submit to us is confidential and can only be accessed by authorized members of our research teams. There is a minimal risk of a loss of confidentiality. We are committed to protecting your information, and your privacy is our top priority.

Once enrolled in the CRR one of our researchers will determine if you are a good fit for their studies based on specific study criteria. If appropriate, they will proceed to contact you and tell you all about their study, answer your questions, and let you decide if you are interested in participating. Participation is totally voluntary.

If at any time you change your mind and no longer wish to be included in our Clinical Research Registry (CRR), simply send an email to or call 312-238-2892, and your information will be removed from the registry and you will no longer be contacted about research participation through the registry. If you decide not to continue with participating in the CRR and your information is removed, you will not be penalized in any way or lose any benefits to which you are entitled, specifically any present or future medical care. While we can remove you from the CRR registry, we cannot guarantee that contributions to past research can be withdrawn.

By submitting information through our website you agree and confirm that you are 18 years of age or older and submitting information about yourself, or you are the parent or legal guardian of a person under 18 years of age on whose behalf you’re submitting information.
